ASSUMPTION OF RISK

I understand that participation in martial arts training and related activities at Shiro Tora Kai LLC involves physical activity and inherent risks, including but not limited to:

• Slips, falls, or collisions
• Sprains, bruises, cuts, or broken bones
• Physical contact with instructors or students
• Muscle soreness or physical exhaustion
• Serious injury or, in rare cases, permanent disability or death

I voluntarily choose to participate and fully accept all risks associated with martial arts training and dojo activities.

MEDICAL RELEASE

I certify that I (or my child) am physically able to participate in martial arts training. I understand it is my responsibility to inform the dojo of any medical conditions, injuries, allergies, or limitations before participating.

In the event of an emergency, I authorize Shiro Tora Kai LLC staff to obtain medical treatment deemed necessary. I understand I am financially responsible for any medical expenses incurred.

RELEASE OF LIABILITY

In consideration of being allowed to participate in activities at Shiro Tora Kai LLC, I hereby release, waive, discharge, and hold harmless:

• Shiro Tora Kai LLC
• Its owners, instructors, staff, volunteers, and representatives

from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may occur during participation in dojo activities, whether caused by negligence or otherwise, to the fullest extent permitted by Oklahoma law.

PHOTO & VIDEO RELEASE

I grant permission for Shiro Tora Kai LLC to use photographs or videos taken during classes or events for promotional purposes, including social media, website content, and advertising.

ACKNOWLEDGMENT

I have carefully read and fully understand this Waiver & Liability Release Agreement and Dojo Rules. By signing below, I acknowledge that I agree to abide by all dojo policies and understand that failure to do so may result in disciplinary action or removal from the program.
